Subsequent Examination Registration

End of Semester Even Academic Year 2020/2021

Students of the Faculty of Industrial Engineering who can take the final semester of the academic year 2020/2021 are students with the following reasons:

1. Sick , so it cannot take the exam. Must attach:
   -Original illness letter from a doctor, hospital/clinic equipped with letterhead, original signature, and official stamp (scan of original letter not photocopy)
   -Drug prescription and receipt/receipt/receipt of drug purchases (scan of original letters not photocopy)
   -Results of screenshoot consultation with doctors in online health applications (halodoc, etc.) The date of the examination, so it cannot take the exam from that date.
2. The competition , bringing a dispensation letter/information to take part in the competition.
3. Get a disaster like a natural disaster
4. The close family died , strengthened by attaching a certificate from the authorities (Hospital/RT RW local).
5. Clashes Exams , by attaching evidence of exam cards.
6. Technical problems , so that they cannot take the examination activities and are required to attach evidence of approval from the lecturer (proven by the screen shot of communication with the relevant lecturer are allowed to take the supplementary examination)

The procedures for taking the follow -up examination are as follows:

1. Register for a supplementary exam at the Bit.ly/uas_Susulan , starting on
   Tuesday - Thursday, June 29 - July 1, 2021.
2. Attach the exam card and the above requirements (according to the reasons)
3. The final semester semester of 2021 will be held on
   Monday - Wednesday, 5 - 7 July 2021.
4. The supplementary examination is only conducted 1 time (no additional supplementary exams).
5. Fill in the Following UAS registration form as much as the number of courses to be proposed.

The list of students who meet the requirements to participate in the Field of Fields will be announced on Friday, July 2, 2021 at the Igracias announcement dashboard.
